It is $35 a month and it is optional. It covers the ongoing work of being found: position checks, content that answers what people are actually searching for, and the profile and technical work behind it. Every action is performed by hand, not by a tool. Two of the sites we manage reached the first page of Google within a few months.
Yes. You see the completed website, built with your own content and your own photographs, before anything is published. Nothing goes live until you have looked at it and said so.
